If you would like to obtain a list of licensees licensed by the Nevada State Board of Medical Examiners, you may utilize the form provided on the website in the "Patients & Consumers" section by clicking the Order Licensee List link), or send a written request to the Nevada State Board of Medical Examiners at 1105 Terminal Way, Suite 301, Reno, NV 89502-2144. Please include the type of licensees to be included (Allopathic Physicians (M.D.s), Physician Assistants, Respiratory Therapists or Perfusionists), and advise us as to the manner in which you would like to receive your list (hard copy or via e-email). If you would like the information sent via e-mail, we will provide the file in Excel format (the file may be zipped if it is a large file) by default, unless you request a pdf. Additionally, by default, the list is sorted by licensee last name. If you require a different sort method, please advise (other choices are by specialty, by zip code, by county or by city). Please provide the recipient e-mail address or mailing address, and enclose payment for the list(s) you are ordering. You may pay by check, cashier's check, money order or credit card. The cost of an M.D. list is $75; the cost of a Physician Assistant list is $25; the cost of a Respiratory Therapist list is $50; and the cost of a Perfusionist list is $5.
