ATTENTION:  All Prospective M.D. Licensees 

of the Nevada State Board of Medical Examiners

Pursuant to Nevada law, effective July 1, 2005, all applicants for medical licensure are required to submit a set of fingerprints and agree to a criminal background investigation by the Nevada Division of Investigation and the FBI.  This procedure will not increase the length of time involved in applying for licensure, but it will increase the costs.  Should any felony convictions be discovered, they will be addressed by the Board.  The authorization forms are available on this website in the "Application and Other Licensing Forms" Section.

The Board's License Specialists are available Monday through Friday from 8:00 a.m. to 5:00 p.m. Pacific Time to handle inquiries regarding applications for licensure:

• Should your last name begin with the letter A, B or C, you will be assigned to License Specialist Kristi Stewart 
  (phone extension "228"), who will handle your application for licensure file.
• Should your last name begin with the letter D, E, F, G, H or I, you will be assigned to License Specialist Brandi Matt 
  (phone extension "255"), who will handle your application for licensure file.
• Should your last name begin with the letter J, K, L, M or N, you will be assigned to License Specialist Rebecca Whitesel 
  (phone extension "254"), who will handle your application for licensure file.
• Should your last name begin with the letter O, P, Q, R or S, you will be assigned to License Specialist Brett Canady 
  (phone extension "231"), who will handle your application for licensure file.
• Should your last name begin with the letter T, U or V, or with the letters Wa through Wh, you will be assigned to Deputy Chief of Licensing Carolyn Castleman (phone extension "233"), who will handle your application for licensure file.
• Should your last name begin with the letters Wi through Wz, or with the letter X, Y or Z, you will be assigned to Chief of Licensing Lynnette Daniels (phone extension "223"), who will handle your application for licensure file.
• NON-U.S. CITIZENS, ONLY:  If you are currently licensed to practice medicine in Nevada, but are a non-U.S. citizen, you will be assigned to License Specialist Brandi Matt (phone extension "255"), who will handle your application file with respect to any changes in your immigration status.
