On March 11, 2011, The Nevada State Board of Medical
Examiners approved, accepted and adopted a settlement agreement which
allowed for an order to be entered finding Mr. Forrest violated NAC
630.380(1)(f), NAC 630.380(1)(m) and NRS 630.3062(1), as set forth in Counts
I and II of the Complaint, and ordering that Mr. Forrest receive a public
reprimand; pay a fine of $2,000; reimburse the Board its reasonable costs and expenses
incurred in the investigation and prosecution of the case within six months of the
acceptance, adoption and approval of the settlement agreement by the Board.
Count III of the Complaint was dismissed. |

The Investigative Committee of
the Nevada State Board of Medical Examiners filed a formal Complaint against
Mr. Forrest charging him with a violation of NAC 630.380(1)(f), malpractice;
a violation of NAC 630.380(1)(m) and NRS 630.3062(1), failure to maintain
timely, legible, accurate and complete medical records relating to the
diagnosis, treatment and care of a patient; and a violation of NAC
630.380(1)(l) and NAC 630.230(1)(L), writing prescriptions for controlled
substances to treat acute pain or chronic pain in a manner that deviates
from the guidelines set forth in the Model Guidelines for the Use of
Controlled Substances for the Treatment of Pain, adopted by the Board. |
