FORMAL DISCIPLINARY ACTION TAKEN BY THE NEVADA STATE BOARD OF MEDICAL EXAMINERS:

JUNE 2, 2001
The Investigative Committee of the Nevada State Board of
Medical Examiners filed a formal complaint against
Dr. Rasmussen alleging that he failed to report the suspension
of his Wisconsin medical license, and the stayed revocation
and probation of his California medical license to the Board.

DECEMBER 19, 2001
The Board ordered Revocation of Dr. Rasmussen's license to
practice medicine in the state of Nevada. The Board also
orderd Dr. Rasmussen to pay all costs incurred in the
disciplinary proceedings within sixty days.
