FORMAL DISCIPLINARY ACTION TAKEN BY THE NEVADA STATE BOARD

The Nevada State Board of Medical Examiners having
reasonable question as to the skill and safety of patients
and the competence of Dr. De Lee Ordered that
he shall submit to an examination testing his
professional competency to practice medicine.

JUNE 5, 1992
The Nevada State Board of Medical Examiners filed a
formal complaint against Dr. De Lee alleging that he failed
an examination to determine his competency to practice
medicine.

JUNE 5, 1992
The Nevada State Board of Medical Examiners Ordered
to Summarily Suspend Dr. De Lee's license to practice
medicine in the state of Nevada pending proceedings for
revocation or othe action.

AUGUST 11, 1992
Dr. De Lee petitioned the Board for restoration of certain
restricted privileges. It was Ordered that Dr. De Lee's
license remains Summarily Suspended except as modified
by the Board. The modifications are as follows, Dr. De Lee
may assist in surgical and delivery procedures performed at
Woman's Hospital only and for certain OBGYN surgeons only,
the Chief of Staff shall provide the Board with a report of
Dr. De Lee's actions at the hospital each month, Dr. De Lee
shall take and pass SPEX , he shall complete sixty hours of
CME, he shall be provided by the Board with a peer
examination of his medical competency by 2 board certified
OBGYNs who are not familiar with the Dr. De Lee.

JANUARY 16, 1992
The Nevada State Board of Medical Examiners accepted the
Stipulation for Settlement and it was Ordered that Dr. De Lee
retire from active practice in the state of Nevada effective
immediately, he shall publish in one newspaper a notice of
his retirement, if he should apply for active licensure he
will be required to meet all the current licensure requirements
for the state of Nevada, and all charges on file against Dr. De Lee
in this administrative action are dismissed with prejudice and no
further action will be taken.
