DISCIPLINARY ACTION TAKEN BY THE NEVADA STATE BOARD OF MEDICAL EXAMINERS: ********************************************************************* JANUARY 25, 1995 The Investigative Committee of the Nevada State Board of Medical Examiners filed a formal complaint against Dr. Galvez based on action taken against his medical license in the state of Wisconsin and his failure to report that action to the Nevada Board within 30 days and attempting to renew his Nevada license by false, misleading, or incomplete statements.
