The Investigative Committee of the Nevada State Board of Medical Examiners filed a
formal complaint against Dr. Hamwi for a
violation of NRS 630.3062(1) for failing to maintain timely, legible, accurate and complete medical records relating to the diagnosis, treatment and care of a patient;
and a violation of NRS 630.306(9), for failure to comply with the requirements
of NRS 630.254, to maintain a permanent address with the Board to which all communications from the Board to the
licensee must be sent and to notify the Board of a new permanent address within thirty days after
a change. |

On February 6, 2009, the Nevada State Board of Medical Examiners (Board)
entered its Findings of Fact, Conclusions of Law and Order, whereby Dr. Hamwi was found guilty of violating NRS 630.3062(1),
for failing to maintain timely, legible, accurate and complete medical records relating to the diagnosis, treatment and care of a
patient; and guilty of NRS 630.306(9), for failing to maintain a current permanent address with the Board. The Board Ordered that
Dr. Hamwi's license to practice medicine is hereby suspended until further Order of the Board;
Dr. Hamwi shall reimburse the Board all costs and fees incurred in the amount of $4,757.56,
and he shall pay a fine in the amount of $2,000.00 within one year from the date of service of the Order. |

The Board granted Fadi Hamwi,
M.D.'s request that the Board reconsider its suspension of his license to
practice as a physician in the State of Nevada. The Board ordered that all
factual and legal findings from the original Order are stipulated and
affirmed, except that the suspension of Respondent's license to practice as
a physician in Nevada is hereby redacted from the original Order. |
