A Settlement, Waiver and Consent Agreement was approved and accepted by the Nevada State Board of Medical Examiners (Board),
whereby Dr. Cook, pursuant to the Settlement, Waiver and Consent Agreement, entered an order finding that the
Dr. Cook violated NRS 630.301(3), in that his California medical license to practice medicine had been revoked, with the revocation stayed contingent upon compliance with terms of probation. The Board ordered that the
Dr. Cook's Nevada medical license be revoked, with the revocation stayed, that he receive a Public Reprimand, that he comply with the terms of his probation in California and that he reimburse the Board's costs and expenses incurred in the investigation and prosecution of the case against him, in the amount of $1547.72, payable within 60 days of the approval and acceptance of this Settlement Agreement. |
