The Investigative Committee of the Nevada State Board of Medical Examiners filed a formal
complaint against Dean Mondell, M.D. on August 7, 2006, for care rendered to a sixty-two
year old female patient. As per Count I, Dr. Mondell allegedly violated NRS 630.301(4), by
failing to provide reasonable care, skill, or knowledge ordinarily used under similar
circumstances. As per Count II, Dr. Mondell allegedly violated NRS 630.3062(1), by failing
to maintain timely, legible, accurate and complete medical records relating to the
diagnosis, treatment and care of a patient.

A Settlement, Waiver and Consent Agreement was approved and accepted by the Nevada
State Board of Medical Examiners, whereby, Dr. Mondell, by not contesting, agreed that an
Order be entered finding that he committed a violation of the Medical Practice Act of the
State of Nevada to wit: one count of malpractice, when he deviated from the standard of
care when he prescribed pain relievers in an amount that resulted in the patient taking
excessively high levels of acetaminophen daily. Dr. Mondell shall, at his own
expense, attend, within one year of the adoption of this Agreement with the Board, six (6)
hours of continuing medical education pre-approved by the Chairman of the Investigative
Committee, in addition to any other continuing medical education required as a condition
of licensure. Dr. Mondelll shall also reimburse the Board $ 1,674.11 for the costs and
expenses incurred during the investigation and prosecution of this case within ninety (90)
days of the acceptance, adoption, and approval of this Agreement. Furthermore, Count II of
the complaint shall be dismissed.
