GILBERT, WARREN S., M.D.

OCTOBER 19, 1992
The Nevada State Board of Medical Examiners filed a formal
complaint against Dr. Gilbert alleging continual failure,
violation of the standards of practice, and repeated malpractice,
writing precriptions for controlled substances in excessive
amounts and precribing certain medications for weight control.

JANUARY 16, 1993
Dr. Gilbert entered into a Stipulation for Settlement with
the Board and it was ordered that he pay a $5,000 fine,
take CME courses on prescribing controlled substances,
be issued a Written Public Reprimand, and be placed on
probation for 2 years.

APRIL 11, 1995
The Investigative Committee of the Nevada State Board of
Medical Examiners filed a formal complaint against Dr. Gilbert
alleging gross malpractice, delegating responsibility for the
care of a patient to a person who is not qualified, aiding or
assisting any unlicensed person to engage in the practice of
medicine, and willfull failure to comply with an Order of the
Board which resulted in the death of a patient.

DECEMBER 18, 1995
The Board ordered to release Dr. Gilbert from the 2 year
probation imposed by the January 16, 1993 order.

JANUARY 17, 1996
The Board ordered to revoke Dr. Gilbert's license, they
stayed the revocation and placed him on 4 years
probation with terms and conditions. He must pay the
cost of the investigative fees, shall document his personal
direction regarding the care of inmates, shall ensure all
physical examinations will be done by licensed personnel,
shall provide a written assessment on a quarterly basis of
deficiencies observed by the jail or prison and what corrective
action will be taken to address these deficiencies.

APRIL 11, 2000
The Board ordered to remove Dr. Gilbert from probationary
status and restored his license to practice medicine in the
State of Nevada to full and unrestricted status.
