DECEMBER 11, 1987
The Nevada State Board of Medical Examiners filed a formal
comlpaint against Dr. Evangelista on the grounds of two
counts of income tax evasion in the state of Illinois, suspension
of his Illinois medical license, and obtaining maintaining or
renewing a license by false, misleading or incomplete statements.
JUNE 29, 1988
The Board Ordered that Dr. Evangelista's license to practice
medicine in the state of Nevada be revoked and the revocation
stayed and he be placed on probation for 5 years. He must obey
all laws including those related to the practice of medicine. It is
also ordered that his license to practice medicine in the state of
Nevada be suspended for a period of six months commencing
from the effective date of this order.
AUGUST 3, 1993
The Board Ordered that Dr. Evangelista be released from probation
and his license be free and clear of any and all conditions and
limitations.
MARCH 25, 1997
The Investigative Committee of the Board of Medical Examiners
filed a formal complaint against Dr. Evangelista alleging failure to
maintain medical records, failure to make medical records available
for inspection, engaging in conduct intended to deceive, writing
prescriptions for controlled substances in excessive amounts,
writing prescriptions for controlled substances without an examination,
gross or repeated malpractice, and failure to use reasonable skill, care or
knowledge.
JUNE 12, 1997
The Investigative Committee of the Board filed a formal
Amended Complaint against Dr. Evangelista. The amended
complaint ADDED one allegation to the Original Complaint,
prescribing appetite suppressants without obtaining a medical
history or performing a physical examination.
SEPTEMBER 25, 1997
The Board Ordered that Dr. Evangelista's license to practice medicine
in the state of Nevada be Revoked.
