OCTOBER 25, 1993
    The Investigative Committee of the Nevada State Board of
    Medical Examiners filed a formal complaint against
    Dr. Chomiak alleging possible impairment and habitual
    dependency upon the use of alcohol and/or controlled
    substances.

OCTOBER 25, 1993
    The Nevada State Board of Medical Examiners Ordered
    to Summarily Suspend Dr. Chomiak's license to practice
    medicine in the state of Nevada.

JANUARY 21, 1994
    Dr. Chomiak entered into a Stipulation for Settlement with
    the Board. It was Ordered that Dr. Chomiak receive a public
    written reprimand, continue therapy with a therapist approved
    by the Investigative Committee for a period of six months after
    the entry of the order and submit to random analyses of body
    fluids collected by the Board.

JANUARY 22, 1994
    The Board accepted the Stipulation for Settlement and the
    Summary Suspension Order was lifted and Dr. Chomiak's
    license to practice medicine in the state of Nevada was restored.
   
APRIL 22, 1997
    The Investigative Committee of the Nevada State Board
    of Medical Examiners filed a formal complaint against
    Dr. Chomiak alleging inability to practice medicine with
    skill and safety because of illness, mental or physical condition
    or the use of alcohol and /or controlled substances, and
    that he rendered professional services to a patient while
    under the influence of alcohol and /or controlled substances.

APRIL 24, 1997
    The Board Ordered to Summarily Suspend Dr. Chomiak's
    license to practice medicine in the state of Nevada until
    further order of the Board.

JULY 15, 1997
    The Board Ordered that Dr. Chomiak's license to practice
    medicine in the state of Nevada be REVOKED.
